Webinar Objectives:
✅ To explore how recent innovations are changing HCC transplant selection and management.
✅ To use two challenging cases to demonstrate contemporary MDT decision-making.
✅ To discuss the evolving role of downstaging, systemic therapy and immunotherapy before LT.
✅ To examine whether tumour biology and treatment response should increasingly complement conventional anatomical criteria.
✅ To discuss controversies and unanswered questions surrounding transplantation beyond current criteria.
Case 1 – HCC Beyond Conventional Transplant Criteria
A patient initially considered outside standard transplant criteria, exploring:
-
Modern downstaging strategies
-
Integration of locoregional and systemic therapy
-
Assessment of biological response
-
When should a patient be reconsidered for transplantation?
-
Should successful downstaging change transplant eligibility?
Case 2 – HCC and the Emerging Role of Immunotherapy Before Transplantation
A challenging patient treated with contemporary systemic/immunotherapy, exploring:
-
Immunotherapy as bridging/downstaging therapy
-
Tumour response and selection for transplantation
-
Timing and washout before LT
-
Risk of acute rejection and graft loss
-
How should MDTs approach these patients in 2026?
MDT Discussion
Following each case, the moderators will lead a structured discussion:
“Would you transplant this patient?”
The discussion will focus on:
-
Tumour biology versus anatomical criteria
-
Response to treatment as a selection tool
-
Limits of current transplant criteria
-
Donor risk and expected recipient benefit
-
DDLT versus LDLT
-
Ethical considerations when expanding transplant eligibility
-
What should constitute sufficient evidence to proceed?
